Funding Opportunity - Request for Proposals

Sturdy Health is pleased to announce approximately $2.6 million in funding available to local community organizations and initiatives focused on:

• Increasing Health and Social Care Access 
• Increasing the Prevention and Management of Chronic Conditions
• Expanding Affordable, Stable, Safe, and Accessible Housing 
• Reducing Social Isolation among Older Adults

Grants have been awarded to the following organizations:

• Attleboro Public Schools - The Attleboro Early Access & Family Stabilization Initiative will strengthen early identification and referral pathways for students and families by connecting schools with community-based behavioral health, social services, and basic needs supports, helping to reduce social isolation and increase participation in supportive community activities.
• Hockomock Area YMCA - The Social Action Services Center and Pathways Forward Program will provide a centralized, community-based navigation and prevention model offering screening, health education, and coordinated referrals to address food insecurity, housing stability, behavioral health access, and social isolation.
• The CHANCE Project - An innovative collaborative initiative uniting six community partners, including the Literacy Center, Attleboro Norton YMCA, Attleboro Public Schools, Attleboro Interfaith Collaborative, The Arc of Bristol County, and Attleboro Council on Aging, to embed community health workers and peer health navigators across community sites to expand access to screening, navigation, transportation, and early intervention for individuals and families.
• NeighborWorks Housing Solutions – The establishment of a “no wrong door” housing access hub in partnership with the City of Attleboro that will provide housing navigation, stabilization services, and financial counseling to reduce housing instability and homelessness risk.
• Manet Community Health Center - For the Health Benefits and Enabling Services Program to expand multilingual health insurance navigation and care connection services, in partnership with Community Counseling of Bristol County, ensuring residents can access and maintain continuous primary, specialty, and behavioral health care.

This community investment from Sturdy Health was made available as part of a statewide Determination of Need process for the expansion of the Emergency Department at Sturdy Memorial Hospital in Attleboro, MA. The funds will be granted to community organizations in early 2026 and may be used over the next three years. Sturdy Health will prioritize projects that focus on communities currently served by the health system, including Attleboro, Foxborough, Mansfield, North Attleboro, Norton, Plainville, Raynham, Rehoboth, Seekonk, and Wrentham in Massachusetts.

The funding priorities were shaped by Sturdy Health’s Community Health Needs Assessment and input from local stakeholders. Grant requests will also be reviewed with a health equity lens to ensure these investments benefit those who need them most.

Note: Applicants must be a not-for-profit organization, tax-exempt under section 501(c)(3) of the Internal Revenue Code. Sturdy Health prioritizes applicants with offices in and/or that currently serve Sturdy Health’s service area.  Hospitals, Academic Medical Centers, and other inpatient acute care facilities are not eligible.
