A Success Story: From Food Insecurity to Holistic Support
One of our recent success stories showcases the outstanding work of Lauren Moriarty, Population Health Associate. Her dedication to patient advocacy and problem-solving has already made a meaningful impact.
A patient was facing significant challenges related to food insecurity and housing. Without access to a functioning microwave, preparing meals was nearly impossible, further complicating his ability to maintain a nutritious diet. Understanding that even small interventions can have a big impact, Lauren worked diligently to find a solution. She reached out to Home Depot, who not only provided a brand-new microwave but also connected her with New Englanders Helping Our Veterans, an organization that assisted in repairing the patient’s home.
This simple yet powerful act of kindness helped build trust with the patient, who then became more open to additional resources. With this newfound trust, Madeleine Davies, PharmD, was able to coordinate a nutritional meal program and a prescription delivery service, ensuring the patient had consistent access to both food and medication. This holistic approach to care has set the foundation for long-term health improvements.
