CHNA Reporting
A Community Health Needs Assessment (CHNA) is a systematic process involving the community to identify and analyze community health needs. The process provides a way for health systems like Sturdy Health to partner with the communities they serve to prioritize health needs, and to plan and act upon unmet community health needs.
Completing a CHNA every three years allows us to keep a pulse on the public health concerns facing our friends, family and neighbors who live within our official service area and develop strategies to address them.
Sturdy Health’s Board of Managers approved the 2025 Community Health Needs Assessment and Implementation Plan for FY2026-FY2028 on October 27, 2025.
