Your Medical Records
At Sturdy Health, it’s important that our patients have access to their medical records and information, both easily and securely. Your records are available through the Health Information Management department located on the ground floor of Sturdy Memorial Hospital. Additionally, you have access to your information through a secure online patient portal.

About The Portal
The Health Information Management department is available to provide information from your medical record at Sturdy Memorial Hospital only. Download the Authorization to Release Health Information document and bring it to the hospital or fax it to (508) 342-1900.Â
Note: Copies of birth certificates may be obtained from Attleboro City Hall.Â
