Remote Support and Monitoring with Brook Health
The primary care providers at Sturdy Health partner with Brook Health for remote nurse care and patient monitoring tools for patients with select health conditions. To help patients manage select health conditions, Sturdy PCPs may refer patients to Brook Health for support between office visits.
The program includes:
- A remote monitoring tool mailed directly to your home
- Regular check-ins with a remote nurse by phone to answer questions and help keep you on track
- Access to remote nursing care at any time if you have questions or concerns about your readings once you begin monitoring
What is Brook Health Remote Care?
Brook Health provides remote nurse care and patient monitoring tools. You’ll be matched with a skilled and knowledgeable nurse at Brook Health. Based on your health needs, you may receive a blood pressure cuff or glucose monitor. Your device automatically sends readings to your Brook Health nurse and your Sturdy Health care team, so they can make sure your care plan is working and your health is improving.

Have a question? Text your Brook Health nurse directly and get answers when you need them — no waiting for appointments or phone calls.
Daily Monitoring
Smart devices automatically send your health readings to your Brook Health nurse and Sturdy Health care team. No manual logging or complicated setup required.
Ongoing Personal Support
You’ll be matched with a Brook Health nurse, nutritionist, and/or health coach who check in regularly, answer questions, and help you stay on track by text or phone.
Fewer Disruptions
Brook Health can help coordinate the supplies you need, like test strips or lancets, so you can focus on feeling better, not managing logistics.
Proactive Care
Your care team gets alerts if anything looks off. That means faster support and peace of mind knowing someone’s looking out for you.
